What BMI actually is
Body Mass Index is just arithmetic: your weight in kilograms divided by the square of your height in meters. It was developed in the 1830s by a Belgian mathematician, Adolphe Quetelet, who was studying population averages — not individual health. It only became a clinical screening tool in the 20th century because it's cheap, fast, and correlates reasonably well with body fat across large populations.

Why doctors still use it
Despite its flaws, BMI remains useful as a first-pass screening tool because it's fast, free, and requires no special equipment. At a population level, higher BMI does correlate with higher risk of conditions like type 2 diabetes and cardiovascular disease. For a routine checkup, it's a reasonable starting point for a conversation, not a diagnosis.
Where it breaks down
BMI can't distinguish between fat mass and muscle mass, which is why it systematically misclassifies certain groups:
- Muscular individuals — athletes and bodybuilders often land in "overweight" or "obese" categories despite low body fat, because muscle is denser than fat.
- Older adults — tend to have more body fat at the same BMI as younger people, because muscle mass naturally declines with age.
- Different body frames — BMI doesn't account for bone structure or where fat is distributed on the body, both of which affect health risk independently of the number itself.
What to pair it with
